The Community Care Clinic has an integrated care model. This means that we look at the whole patient; not just the patient’s physical well-being, but also their emotional, social, mental, environmental and spiritual needs. 

  • Primary Care

    At the Community Care Clinic, we focus on keeping you healthy. We offer many services like yearly check-ups, help with ongoing health issues, and care for sudden problems. Our friendly team is here to help you.

    If you are not sure how to take care of your health, don’t worry! If it’s been a while since you visited a doctor, you can set up your first visit with us. During this visit, you will talk with a caring health provider who will help you make a plan for your health. We are here for you!

  • On-Site Lab

    We want to make healthcare easier for you. That's why we provide free lab services right here in our clinic.

    Our on-site lab services mean you don’t have to go anywhere else for your tests. This is great because you won’t need to worry about how to get to another lab, and you won’t have to pay extra fees to other companies for your lab tests.

  • Mental Health

    Mental health care is an important part of overall health and wellness. It helps us to feel better mentally and emotionally, which is just as crucial as taking care of our bodies.

    Kim Carter, who is a licensed psychological associate (LPA), provides mental health therapy services to patients. This means that she helps people work through their feelings, thoughts, and behaviors in a supportive way. Therapy can be very beneficial, helping individuals cope with stress, anxiety, depression, and other challenges they might face in life.

    To make these services accessible to everyone, Spanish interpretation is available. This means that if you speak Spanish, you can still receive help and support in a language you understand.

  • Orthopedics

    Muscle and joint pain can be uncomfortable and may make it hard for you to do the things you enjoy. Orthopedics is a type of medicine that helps treat these kinds of problems. If you feel pain or stiffness, an orthopedic doctor can help you find ways to feel better.

    It’s important not to let pain stop you from living your life. There are many treatments available that can make you feel stronger and more comfortable. Whether it’s physical therapy, medication, or some other kind of support, help is out there. You deserve to move freely and enjoy your daily activities without being held back by pain.

  • Nutrition

    Creating a healthy diet can be hard, especially if you have other health issues to think about. Eating well is important for keeping our bodies strong and feeling good. Sometimes, it can be tough to know what foods are the best to eat or how to cook them.

    Nutrition counseling is a way to get support and guidance to help you choose healthier foods. A Registered Dietician can work with you to understand your needs and help you make better choices. They can explain things in a simple way, so it’s easier for you to understand.

  • Diabetes Education

    It can be hard to know what you need to do to take care of yourself when you have diabetes. That’s why we have a special education and management program for patients managing diabetes. This program is here to help you understand your choices and make things easier.

    We offer support with changing your everyday habits to feel better and control your diabetes. We also provide information about the medicines you might need to take. Our lab services can check your blood sugar and help you stay on track.

    Additionally, we give out free glucose monitors. These devices let you check your blood sugar levels at home, so you can see how your body is doing. Our friendly team is here for you throughout your journey.

  • Physical Therapy

    Injuries and chronic pain can be expensive and difficult to manage. We offer physical therapy, which is a type of treatment that can assist our patients in feeling better and enjoying life more fully.

    Physical therapy can be a great choice for anyone who wants to recover from recent or past injuries. It is also beneficial for those who want to improve their movement and reduce their pain throughout the day. Our therapists work with you to create a personalized plan that meets your needs.

  • Smoking Cessation

    You don’t have to quit using tobacco or nicotine products by yourself. Our friendly providers are here to help you on your journey to better health.

    Quitting can be tough, but you don’t have to face it alone. Our team can help reduce your cravings and make it easier for you to stop using these products. Together, we can work on a plan that fits your needs.

  • Gynecology

    Gynecological and women’s health services are very important for keeping women healthy. It is crucial to attend your annual check-ups and any other appointments without delay. These visits allow healthcare providers to monitor your health and catch any potential issues early.

    By beginning your care early and getting to know your healthcare provider, you can help reduce feelings of anxiety and discomfort. It's normal to feel nervous about these types of exams or visits, but having a good relationship with your provider can make a big difference. When you are familiar with your provider, you can feel more relaxed and confident if a problem arises.

  • Reduced Cost Vision & Dental Care Referral

    Vision care is offered by Western Carolina Eye Associates. They help you take care of your eyes and vision if you have any problems with your eyesight or need a check-up.

    Dental care is provided by Steven Airey, DDS. He is a dentist who takes care of your teeth and gums. Regular check-ups are important for keeping your mouth healthy.

    Referrals to these resources are available only if you have been a patient at CCC for at least 6 months.

  • Spanish Interpretation

    Spanish Interpretation is provided through the Latino Health Project. Call LHP for assistance with scheduling.

  • Community Resource Referral

    Our Community Resource Center is available to all community members regardless of patient status at the Community Care Clinic.

    The Resource Center is staffed by social work interns from Appalachian State University. Intern availability varies by semester, contact the Community Care Clinic if you would like to make an appointment.
